Appointment Information

A new patient appointment typically takes about 45 - 60 minutes. This includes: a thorough exam, treatment, and creation of a treatment plan.

Athletic or comfortable clothing is preferred. Please come to the appointment 15 minutes ahead of your scheduled time and have your insurance card and a photo ID readily available.

If you are able to print out and complete the New Patient Forms, please do so to bring the completed forms to your appointment.

If applicable, please bring any lab or diagnostic reports from other health care professionals who have treated you for your current condition.

 

Insurance & Patient Responsibility

We are participating providers with most major insurance companies. We collect your insurance information for our records. It is your responsibility as the patient to contact your insurance company for your in-network providers and benefits.

No Insurance, No Problem!

We have self pay options.

We accept all major credit cards, personal checks, and cash. We also accept FSA and HSA accounts.

In the case of an auto accident, workman’s compensation, or personal injury claim, we will work with you to ensure correct reimbursement from either auto insurance or workman’s compensation. Please bring with you ANY documentation, for example, a police report would be appropriate for an auto accident.
